Terms and Conditions:

It is MANDATORY that you provide us with the following documentation AT LEAST TWO WEEKS BEFORE THE WORKSHOP:

A copy of your State Medical License (and Registration, if your state requires one).

A photo ID must be presented at the workshop that matches your medical license.

You agree to complete our Post-Workshop Evaluation in order to receive your CME credit certificate.

In order to participate in the Hand-on Clinical Training, you must complete ALL online training modules and pass each exam. If you have not completed the online materials you will not be allowed to inject on our clinical day.

Cancellation Policy:

In order to receive a full tuition refund you MUST cancel no later than 30 days prior to the scheduled event. Please use the “Contact Us” form to send a written notification of cancellation, or, email us at info@restoracademy.com.
